Passport Services

Moon Library is a US State Department authorized Passport Acceptance Facility. The certified passport agents in the Library are able to distribute forms, accept completed paperwork, review required documentation, and forward applications to the U.S. regional passport agency in Philadelphia PA.

Passport Services Office is located in Moon Library office103A.Hours are 9:00am to 2:00pm Monday – Friday. Appointments are required by contacting Nan Clark at 315-470-6726 or nfclark@esf.edu. Accommodations may be made outside of regular hours.

Appointments are available between 9:00 am and 2:00 pm Monday through Friday and can be made by contacting  Nan Clark at 315-470-6726 or  nfclark@esf.edu. Accommodations may be available outside of regular hours. Again, call for an appointment.

 

When should my Passport arrive?

Passports take 4-6 weeks to process.

What do I need to do?

  1. Complete the proper application forms BEFORE coming to the library. This links to the  form  https://pptform.state.gov/?Submit2=Complete+Online+%26+Print
  2. Schedule an appointment with Nan Clark nfclark@esf.edu 315-470-6726
  3. Appear personally at the arranged time.
  4. Bring the following items to your appointment:
    1. Current valid picture identification (driver's license or State ID card)
    2. Proof of United States Citizenship (original birth certificate, certified copy of birth certificate or original naturalization certificate).
    3. Bring in one passport photo. On the SU campus CVS and the Copy Center in Marshall Square Mall take passport photos
    4. Two checks or money orders to cover the required fees.

Passport Renewals

Passport Renewals

Adults:

If the passport was issued 15 or below prior years, the applicant must process his/ her individual renewal form. Moon Library will supply the Renewal Form (DS-82). ALL Instructions for renewing an adult passport are provided on the back of the renewal form.

If the passport was issued 16 or more prior years, the applicant must use a passport agent.  The library will process the application using the same procedure and fees as a new passport application.

Renewal Fees are the same as a normal Passport Application.

•Adults - Bring prior Passport and valid Driver's License.

Children:

•Children- Bring prior Passport, Original Certified Birth Certificate

◦If 15 years and younger - EITHER both parents must be present or one parent and certified letter from other parent.

If 16 & 17 years - One parent must be present
